Well, if you are still working at the scale indicated by the measurements in the first pic, then it's just too big. The weight of all that cloth is dragging it down, and making it sag more than it should. MD works on a real-world human scale. The fabric properties are based on that scale. My pocket ...

Start with a short sleeve. One that ends where you want the (long) sleeve to be pushed up to. Box-pin the bottom edge area of it to hold it in place. Check your import/export scaling, for both programs. The most common issues when moving between different programs can be traced to having the items at the wrong scale.

One last word on the fabric properties. Density effectively equals weight. (There are real-world reasons for this, but you needn't fret over understanding...) A Particle Distance setting of 5 is really for a final rendering... it creates a mesh that is extremely fine (small polys). Your image looks to be a test run, with a simple piece of cloth. Additionally, judging by the scale of the grid on the ground plane, your cloth is quite large. Both these thing...
